CHECKLIST

 1.   X  Be sure to sign all marked places (2 on application & 1 for witness)

 2.   X  Copy of Business license (eg, Incorporation doc. or license with bus. address on it,

            retail lease agreement can also use utility/phone bill showing bus.   

            name/address)

 3.   X  Copy of voided business check(Temp. Check isnt acceptable unless accompanied  

            by letter from bank(ON BANKS LETTERHEAD) stating that account has been

            recently setup.  This letter only has to be one sentence long and signed by

            bank employee.

 4.____Copy of 3 most recent months credit card processing statements
            (only if currently processing credit cards)

 5.   X  FAX ALL PAGES to: 501-325-9756. 

 6.   X  Please be sure to fax ALL 4 pages of application back together so that they can be

            forwarded to the credit card processing company in one group for approval.

 7.   X  Click WEBSITE SETUP to ensure that your website has all the details that

          Visa/MC require for approval.
